New, Existing and Closed Business Forms
Any business that operates in the Town of Carver must file a Form of List annually with the Assessor's Office. This is the basis for personal property taxes. All businesses that are on file for the previous fiscal year, and any new business from the previous calendar year, will receive the Form of List in early January. These forms are due back to the Assessor's Office by March 1st.

If you have closed your business, please fill out the form below and mail or email it back to our office. If we are not notified of your business closing, it may result in additional personal property tax bills being assessed to your business.
